Order a Repeat Prescription

Patient Services

It is easier and quicker to request repeat prescriptions via our online service. Simply log in and select an option.

Please use this form to order your repeat prescriptions.  Please allow 4 working days for us to process your request prior to when you expect to collect your medications from the chemist shop.  Remember to take weekends and bank holidays into account. 

Your scripts will be sent to your allocated chemist so you do not need to come to the surgery to collect the paper prescription and physically take to your chemist unless it is an urgent script for a controlled drug.  The chemist must always have the paper script in their possession in order to dispense controlled drugs.  

Repeat items can be ordered online up to 7 days in advance of their due date to ensure you always have a supply.  Please don’t leave it to the last tablet to place your order as this may result in you having no medications for a few days

If you forget to request a Repeat Prescription

If you forget to obtain a prescription for repeat medication and run out of important medicines, you may be able to get help from your pharmacy. Under the Urgent Provision of Current Prescribed Medicines and Appliances, pharmacists may be able to supply you with a few days supply of a previously repeated medicine without having to get a prescription from your GP first.

We will not process any prescriptions requests if they are made more than 7 days before the current medication runs out. Exemptions to this would be if you were going on holiday and were likely to run out whilst away. Please let the practice know if this is the case when you are making the request.

In person

You can do this by returning the right-hand half of a previous prescription for the required medications, or by submitting a handwritten request.

Please tick the items required and put it in the prescription box in the entrance foyer (during surgery hours).

By post

You can post your prescription slip or written request to us at the Practice. You must include a stamped addressed envelope for return by post if you will not be able to pick up your prescription from the surgery. (Please allow extra time for any possible delays with the postal service).

Pharmacy Ordering/Collection Service

Pharmacies offer a prescription collection service from our Practice.They can also order your medication on your behalf. This saves you time and unnecessary visits to the Practice. Please contact the pharmacy of your choice for more information if you wish to use this service and also let reception know which pharmacy you have chosen.

Call our dedicated script order line 01355 225374 (option 4) and leave a message with your name, date of birth, name of the medication you require.  Please have the empty packaging to hand when you call, for reference. Please speak clearly.

If you register for online services – see box at top of page – you can order your repeat prescriptions online.

Medication reviews

Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip. Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions. Medication reviews can sometimes be done by telephone consultation if a blood pressure and blood samples are not required.  Each medication is different so please check in case you don’t necessarily need to attend the surgery for a face to face appointment.

Non-Repeat Items (Acute Requests)

Non Repeat Prescriptions known as “Acute” prescriptions are medicines that have been issued by the Doctor but not added to your repeat prescription records. This is normally a new medication issued for a trial period and may require a review visit with your Doctor prior to being added onto your repeat prescription records. Some medications are recorded as acute as they require to be closely monitored by the Doctor. You may therefore not always be issued with your requested medication as you may need to consult with your Doctor.

Hospital Requests

When you are discharged from hospital you should normally receive five days supply of medication.

On receipt of your medication requirements, which will be issued to you by the hospital, please bring this to the surgery or post as soon as possible before your supply of medication has run out.

Hospital requests for change of medication will be checked by the GP first, and if necessary your Doctor will issue you with a Prescription. We still require at least two working days notice to have time to process your prescription.
